- Physical dependence with daily use (1–2 weeks)
- Withdrawal symptoms on cessation
- Drug interactions with CNS depressants, SSRIs, and many prescription medications
- Elevated risks from 7-hydroxymitragynine (7-OH) products vs leaf kratom
- Pregnancy and breastfeeding contraindications including Neonatal Abstinence Syndrome
- Hepatic stress with chronic heavy use, especially combined with alcohol or acetaminophen
